Wrinkle treatments can smooth out your skin, but the results aren’t permanent. As you age, your skin continues to acquire new wrinkles. Most wrinkles appear on the parts of the body where sun exposure is greatest. These especially include the face, neck, the backs of the hands, and the tops of the forearms. Wrinkles come in two categories: fine surface lines and deep furrows. Wrinkle treatments are in general much more effective for fine lines. Deeper creases may require more aggressive techniques, such injection of fillers or plastic surgery.

Skin ages all over the body, but much more so where there has been sun exposure. Changes brought on by sun damage (photoaging) include “dryness” (really roughness), sagginess, skin growths like keratoses (”liver spots”), and wrinkles.
The effects of aging on the dermal layer are significant. Not only does the dermal layer thin, but also less collagen is produced, and the elastin fibers that provide elasticity wear out. These changes in the scaffolding of the skin cause the skin to wrinkle and sag. Also, sebaceous glands get bigger but produce less sebum, and the number of sweat glands decreases. Both of these changes lead to skin dryness.
It is difficult to repair deeply furrowed skin that appears with photoaging, but thinner wrinkles, dark spots, and rough skin may be improved with the use of topical medications. Over the counter creams containing retinol or glycolic acid may help somewhat. Your dermatologist may also recommend more aggressive prescription acid creams or peels, or, prescribe stronger retinoids such as Retin A (tretinoin) or Tazorac (tazarotene.)

A skin graft is thus a skin transplant. Skin grafts are performed by surgeons (including plastic surgeons) and by some dermatologists.
Mohs microscopically controlled excision - Mohs Micrographic Surgery, an advanced treatment procedure for skin cancer, offers the highest potential for recovery—even if the skin cancer has been previously treated. This procedure is state-of-the-art treatment in which the physician serves as surgeon, pathologist and reconstructive surgeon.
Laser Skin Resurfacing - A laser is a high-energy beam of light that can selectively transfer its energy into tissue to treat the skin. The laser makes it possible to change tissue without making an incision. So a surgeon can treat birthmarks or damaged blood vessels, remove port wine stains, and shrink facial “spider veins” without major surgery.
Photodynamic therapy - Applying a chemical called aminolevulinic acid to the skin and exposing the skin to a special light source.
Topical chemotherapy - Applying a chemical such as 5-fluorouracil, diclofenac sodium, or imiquimod to destroy pre-cancerous growths and some cancerous lesions.
Radiation therapy - Using x-rays to destroy tissue in certain types of skin cancer, as well as in selected individuals for whom surgery is not possible.
Dermabrasion - Removing the outer layers of skin and softening irregular edges. After the skin is frozen with a spray medication, a high-speed, rotary abrasive wheel is used.
Cryotherapy (liquid nitrogen) Applying or spraying liquid nitrogen onto the skin to freeze and destroy the tissue.
Precautions and care for Skin Surgery
Your skin growth will be removed surgically in the office and sent for microscopic examination to be certain it has been completely removed. Unless otherwise instructed, please eat normally and take all regularly prescribed medications.
After surgery, most patients may resume their usual activities; sometimes, there are temporary restrictions on sports, dancing, or other physical activity.
Aspirin makes people bleed more easily, and we prefer that patients not take it for seven days before surgery and for two days after surgery.
If you are taking aspirin, or a medicine containing aspirin, on your own, please stop it for seven days before surgery.
Antibiotics - People who have had hip replacements, have artificial heart valves, or who have mitral valve prolapse often have to take antibiotics before dental procedures.

Stitches (sutures) are used to close the wound after surgery. The type of stitch we use depends on the surgery and your skin. Stitches that need removal are usually taken out 7 to 14 days after surgery. Stitch removal and takes only a few minutes.

Causes of Acne Scars
In the simplest terms, scars form at the site of an injury to tissue.
They are the visible reminders of injury and tissue repair. In the case of acne, the injury is caused by the body’s inflammatory response to sebum, bacteria and dead cells in the plugged sebaceous follicle.
Two types of true scars exist, depressed areas such as ice-pick scars, and raised thickened tissue such as keloids.

Basic research into how acne scars are formed and how they respond to treatment is very limited. It has not been a well treated condition in the past.
Prevention and Treatment of Acne Scars
The best way to prevent post-inflammatory changes caused by acne is to prevent acne lesions from occurring. This is done by understanding the factors that cause acne and using the appropriate treatments for the different acne types.
If you have scarring, the acne scar treatment you get will depend on the type of scars you have, as well as on your skin type. Most experience has been gained treating facial scars. Generally, scars on the chest and back do not respond as well.
Collagen injection. Collagen, a normal substance of the body, is injected under the skin to “stretch” and “fill out” certain types of superficial and deep soft scars. Collagen treatment usually does not work as well for ice-pick scars and keloids.
Autologous fat transfer. Fat is taken from another site on your own body and prepared for injection into your skin. The fat is injected beneath the surface of the skin to elevate depressed scars.
Punch Excision - Ice pick acne scars have hard, irregular jagged borders and often the depth is irregular as well. Simple excision of these scars with a sutured closure allows the dermatologist to bring the dermis back together, get rid of the ragged margins and close the area with a fine, uniform line.
Subcision - In this procedure the dermatologist undermines the acne scar with a sharp instrument such as a tiny scalpel or needle.
Subcision helps break fibrous bands of scar tissue that are creating tension between the epidermis and deeper structures and also helps induce new collagen formation.
Dermabrasion - This procedure has fallen somewhat out of favor with the availability of newer, easier to use techniques. In dermabrasion, the skin is anesthetized and frozen and an extremely sharp rapidly rotating blade sheers away damaged tissue.
Laser Treatment. Lasers of various wavelength and intensity may be used to recontour scar tissue and reduce the redness of skin around healed acne lesions.
Skin Surgery. Some ice-pick scars may be removed by “punch” excision of each individual scar. In this procedure each scar is excised down to the layer of subcutaneous fat; the resulting hole in the skin may be repaired with sutures or with a small skin graft. Subcision is a technique in which a surgical probe is used to lift the scar tissue away from unscarred skin, thus elevating a depressed scar.
